Provides leadership to facility through effective organization, direction and appropriate delegation of activities and maintains appropriate oversight of facility functions to ensure smooth and effective operation.

Admissions Coordinator

Coordinate activities related to admission of residents to facility and to coordinate community relation’s activities for the facility.

Business Office Assistant

Assist in the day-to-day accounting functions of the facility in accordance with current applicable federal, state, and local standards, guidelines and regulations.

Business Office Manager

Direct the overall accounting activities in accordance with the current applicable federal, state and local standards, guidelines and regulations. These activities include but are not limited to accounts receivable, accounts payable, human resources including payroll.

Case Manager

Promote effective utilization of skilled nursing and rehabilitation services. Facilitate clinical coordination among all members of the interdisciplinary team. Be the liaison between the resident/family members, facility staff and the payer in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility.

Assist in providing supplies and equipment in an efficient manner, in accordance with current applicable federal, state, and local standards, guidelines and regulations and as may be directed by the Director of Nursing Services, to assure that the highest degree of quality resident care can be maintained at all times.

Certified Nursing Assistant (CNA)

Provide each residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors.

Dietary Aide

Provide assistance in all dietary functions as directed by your supervisor and in accordance with established dietary policies and procedures.

Dietary Cook

Prepares food in accordance with current applicable federal, state and local standards, guidelines and regulations.

Dietary Service Supervisor

Supervises the operation of the dietary department daily to assure quality meal preparation, menu and diet compliance, interview resident or family members to obtain diet history, completes initial assessment of all new residents in cooperation with Registered Dietician, etc.

Directs the overall operation of the nursing department and the delivery of care to the residents in compliance with current federal, state and local standards, guidelines, regulations and laws that govern the facility.

DSD

Coordinates all education needs of all employees in accordance with local, state and federal regulations. This is a position overseeing and leading the educational efforts of the facility, this position is not a supervisory role over the nursing staff including CNAs or licensed nurses.

Housekeeping

Perform the day-to-day activities of the Housekeeping Department in accordance with current Federal, State, and local standards, guidelines and regulations governing our facility and as may be directed by the Administrator, and/or the Director of Housekeeping, to assure that our facility is maintained in a clean, safe, and comfortable manner.

Laundry

The primary purpose of your position is to plan, organize, develop and direct the overall operation of the Laundry Department in accordance with current Federal, State and local standards, guidelines and regulations governing our facility. As may be directed by the Administrator, to assume that our laundry department is maintained in a clean, safe and sanitary manner and that an adequate supply of laundry/linen is on hand at all times to meet the needs of the residents.

Licensed Vocational Nurse (LVN)

Direct nursing care to the residents and to supervise the day-to-day nursing activities perform by the certified nursing assistants.

Ensure that maintenance schedules are followed as outlined for respective shift or area, report all accidents/incidents to your supervisor no matter how minor they may be. (NOTE: Such occurrences must be reported on the shift in which they occur.)

Marketing

Targets and delivers marketing presentations to physicians, hospital discharge planners and community-based organizations on a regular basis, lead and coordinate activities of the facility marketing team, whose goal is to assist in the development of the facility marketing strategy and develop and implement quarterly facility marketing plans with goals and objectives to meet/exceed budgeted census.

Medical Record Designee

Maintains medical records in accordance with federal and state guidelines as well as in accordance with facility established policies and procedures, to assure that a complete medical records program is maintained.

Nurse Consultants

Assist the facility in proactive development of QI plans, act as a liaison between the facility and Resource Center, To act as a resource to the facility and be available for questions, concerns, and needed assistance assist the facility in maintaining regulatory compliance, as well as compliance with company policies, develop IDT with a goal of facility independence, assist in education regarding new policies, standards of care, and regulatory requirements, assist facility in identifying priority focus areas and collaborate with VPO's to meet the facilities clinical and financial goals.

RCC

Provides each residents with daily routine nursing care and services in accordance with the resident’s assessment and care plan. As may be directed by supervisors.

Answers phone and direct calls to appropriate staff, greets visitors and provide direction and assistance as needed.

Regional Vice President

Provides daily oversight to the facilities as well as to oversee and improve the profitability of the facilities.

Registered Nurse (RN)

Provide direct nursing care to the residents and to supervise the day-to-day nursing activities perform by the certified nursing assistants. All care and supervision must be in accordance with current federal, state, and local standards, guidelines, regulations and laws that govern our facility.

Registered Nurse (RN) Supervisor

Supervise the day-to-day clinical activities of the facility to ensure that the highest quality of care is maintained at all times. Such supervision must be in compliance with all current federal, state, and local laws, rules, regulations, and guidelines.

RNC

Perform restorative nursing procedures as ordered by the physician and determined by the resident’s assessment and care plan, and as director by your supervisors.

Plan, develop, organize, implement, evaluate and direct social service programs in accordance with current existing federal, state, local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
